On or about June 4, 2014 at residential structure located at 202 Lucia Ln, Maumelle, AR 72113, the employer did not ensure an employee performing roofing operation on a one story residential structure was properly protected by the use of safety glasses while using the pneumatic nail gun. The employee was observed exposed to eye impalement hazards.

On or about June 4, 2014 at residential structure located at 202 Lucia Ln, Maumelle, AR 72113, the employer did not ensure an employee used fall protection systems while working on a roof. The employee was observed working in proximity to the direct edge of the roof and was exposed to falling from the roof edge. The employee was exposed to slipping and elevated fall hazards. Affordable Residential Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 936678, citation number 1, item number 1 and was affirmed as a final order on 04/02/2014, with respect to a workplace located at or near 9 Greymoss Cove, Little Rock, AR 72211.
