29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:    a) Production Area- The employer failed to ensure that electrical equipment, such as the Belkin 7-outlet  relocatable power tap that was powering two extension cords, two fans, a radio, and Thumb Slot Slicers 0179, 0361, and 0362, was being used only to power audiovisual equipment as permitted under its listing and labeling.      b) Production Area- The employer failed to ensure that electrical equipment, such as the 3-outlet  relocatable power tap that was powering two additional 3-outlet relocatable power tap cords which then powered the Sleeve Cutters at stations H1 and H2, was being used only to power audiovisual equipment as permitted under its listing and labeling.       No abatement certification or documentation is required for this item.

29 CFR 1910.334(a)(3)(i): A flexible cord used with grounding type equipment did not contain an equipment grounding conductor:    a) Production Area-  The employer failed to ensure that all pieces of portable electrical were equipped with a means of grounding the electrical circuit through the use of a grounding pin, such as the TPI Corp. Industrial Fan, Serial #488929, that was observed with a plug attachment that was missing the grounding pin.      No abatement certification or documentation is required for this item.
