Chadwick St, Philadelphia, PA - The employees had worked on a fabricated framed scaffold 3 sections high which did not have proper access for the employees to reach the various working levels of the scaffold thus exposing the employees to a fall hazard. The condition was observed on My 23, 2014.

Chadwick Street, Philadelphia, PA - The employees had worked on a fabricated framed scaffold 3 sections high within 3 feet of the energized 120/208 volt electrical power lines. This hazard exposed the employees to a shock/electrocution hazard. The condition was observed on My 23, 2014.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold more than 10 feet above a lower level was not protected from falling to that lower level by use of personal fall arrest systems or guardrail systems: a) 760 S Chadwick Street, Philadelphia, PA - The employees were working off of a fabricated frame scaffold approximately 19 feet long, 19 feet high and 5 feet wide which did not have a guardrail system installed on the working levels of the scaffold. This hazard exposed the employees to a fall hazard ranging from approximately 13 feet to 19 feet. This condition was observed on May 23, 2014
