Main Street, Manville, NJ 08835: Employees were allowed to perform stucco finishing/painting operations from a fabricated frame scaffold approximately 12' 10" from top level and 7'6" from the middle level to the ground below that was not fully planked from the front upright to the rear upright. Condition observed on or about August 23, 2014.

1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access: A) 20 N. Main Street, Manville, NJ: Employees were exposed to fall hazards of approximately 12' 10" from top level and 7'6" from the middle level to the ground below when a ladder was not provided for safe access to a fabricated frame scaffold. Condition observed on or about August 23, 2014.
