29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    a.) The Timken Company located at 2325 E. Mansfield Rd. Bucyrus, OH 44820; On or about June 3, 2014, an employee was exposed to a crush injury while clearing misaligned parts on the 9-Line cone automatic assembler without utilizing the energy control procedure.  he employee sustained a crush injury to the right hand from a pneumatic ram which was not deenergized.  The work performed by the employee (clearing of the misaligned parts) was not a minor tool change or adjustment within the lockout exception, and so energy control procedures were required.    b.) The Timken Company located at 2325 E. Mansfield Rd. Bucyrus, OH 44820; On or about June 3, 2014, the employer did not ensure employees utilized energy control procedures when entering the 5 Line assembly area to remove and replace nest assemblies.  Removing and replacing nest assemblies for purpose of making adjustments are not minor tool changes or adjustments within the lockout exception, and so energy control procedures were required.

29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s):  a.) The Timken Company located at 2325 E. Mansfield Rd. Bucyrus, OH 44820; On or about June 3, 2014, the employer did not ensure that energy isolating devices needed to control hazardous energy of the 9 Line assembly press were operated, so as to isolate hazardous energy sources while employees cleared jams.  On April 11, 2014, an employee, while clearing the press table due to a jam, was exposed to uncontrolled hazardous pneumatic energy that resulted in a crush injury to the hand.  b.)  The Timken Company located at 2325 E. Mansfield Rd. Bucyrus, OH 44820; On or about June 3, 2014, the employer did not ensure that energy isolating devices needed to control hazardous energy of the 5 Line assembly area were operated while employees entered the line's cell to perform tasks associated with rebuilding nest assemblies.  Employees that entered the hazard cell were exposed to caught-by and struck-by hazards inside the cell in that the equipment was not deenergized.
