29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees:    (a): Cle Elum Flume, flood gates were closed and locked by supervisor and foreman to prevent water from entering the canal while repair work was being performed. A group lock box was provided however employees authorized to work in the canal were not allowed to place their locks on it for their protection.
