29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: Location: 11251 Montgomery Rd, Cincinnati, OH, On or about June 5, 2014, Two employees working for D.M. Norris Company, Inc. were observed working at the edge of steep sloped commercial roof cleaning gutters without the use of any personal fall protection equipment exposing the employees to a fall hazard greater than 11 feet.
