29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Employees were exposed to amputation hazards while operating the following equipment and the employer did not ensure adequate machine guarding was provided to protect the operator and other employees: a) The General Taper Gluer HL051030E side guard was not utilized, which enclosed the power driven rollers. b) The 4037 Flexo Langston 2012 was missing side guards, which enclosed the power driven rollers. c) The Tecasa Juniper 66x126 S#JU-3P-TU-ST-32/2536 had the interlock disabled, allowing access to rotating machine components. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.219(f)(3): Sprocket wheels and chains, which were seven (7) feet or less above floors or platforms, were not enclosed: Employees were exposed to amputation hazards in the Production Area due to the General Taper Gluer HL051030E having exposed chains that extended approximately 62 inches from the ground and the employer failed to enclose the chains. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
