29 CFR  1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation.  a) On or about September 6, 2023, the employer did not ensure that employees engaged in maintenance activities on the Flexo 1 Langston 253 (SN S661159) machine, received training in the recognition of applicable hazardous energy sources and the methods and means necessary for isolation and control.  Employees were exposed to amputation and crushing hazards when placing their hands inside machine to clear jams.   Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1910.147(d): Application of control.  The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:  a) On or about September 6, 2023, the employer did not ensure that employees implemented energy control steps prior to performing maintenance activities such as unjamming boxes on the Flexo 1 Langston 253 (SN S661159) machine.  Hazardous energy was not isolated prior to employees engaging in servicing and maintenance activities, exposing employees to crushing and amputation hazards.  As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine hazards.   Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) Production Area:  On or about January 17, 2024, employees were exposed to crushing, laceration and amputation hazards when operating the Flexo 1 Langston 253 (SN S661159) machine.  Employees would operate machine with no guarding in place to prevent employees from getting their hand/fingers/arms caught between the machine rollers or conveyor box feeder in the stacker section of the machine.      Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1910.1200(e)(1):The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  a) On or about January 17, 2024 Cross Container Corporation did not develop or implement a written hazard communication program in accordance with 29 CFR 1910.1200(e)(1) that would describe or include at least the following:  1) Requirement for labeling of containers of hazardous chemicals; 2) Material safety data sheet availability; 3) Training of employees; 4) A complete list of hazardous chemicals known to be in the workplace; 5) Methods to inform employees of the hazards on non-routine tasks; and 6) Methods to inform other employer(s) of safety data sheet availability; the labeling system; and any precautionary measures to protect employees.  Employees were exposed to hazardous chemicals, including petroleum distillates and liquid propane gas (LPG).  Abatement documentation is required for this item in accordance with 29 CFR 1903.23(d)
