29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  (a)  Water/Waste Office;  A white relocatable power tap was plugged into a black extension cord devise with an outlet box. The UL listing states that relocatable power taps are designed to be plugged into an outlet, and not daisy chained and plugged into other relocatable power taps or extension cords.  This exposes employees to possible electrical fire hazards.    (b)  Business Specialist Office; A Bella one cup coffee pot was plugged into a relocatable power tap. The UL listing states that relocatable power taps are designed for low amperage computer and audiovisual types of equipment, and not appliances.  This exposes employees to possible electrical fire hazards.    (c)  A white relocatable power tap was plugged into another white relocatable power tap. The UL listing states that relocatable power taps are designed to be plugged into an outlet, and not daisy chained and plugged into other relocatable power taps or extension cords.  This exposes employees to possible electrical fire hazards.    The USDA, Rural Development was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 948706, citation number 1, item number 1and was affirmed as a final order on December 6, 2013, with respect to a workplace located at Olympia, Washington

29 CFR 1910.303(g)(1)(ii): The required working space about electric equipment rated 600 volts, nominal, or less to ground, was used for storage:  a)  Furnace Room;  The breaker panel for the office was not accessible due to storage of boxes and a TV cart.
