29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a.  On or about June 5, 2014, on the east side of a framed house, an employee working beneath employees building a framed structure was not wearing a hard hat, exposing the employee to the hazard of being struck by falling debris.     a.  On or about June 5, 2014, on the south side of a framed house, an employee working beneath employees building a framed structure was not wearing a hard hat, exposing the employee to the hazard of being struck by falling debris.

29 CFR 1926.451(b)(2): Scaffold platforms and walkways were not at least 18 inches wide:  a.  On or about June 5, 2014, on the north side of the house, an employee installing framing was using 2 inch by 10 inch wood planks as a scaffold platform, exposing the employee to an 8 feet, 5 inch fall hazard.

On or about June 5, 2014, on the north side of the house, the carpenter's scaffold did not have adequate firm foundations, exposing employees to a 8 feet 4 inch fall hazard.

On or about June 5, 2014, on the second level of a framed house, employees installing framing material were not protected from a 16 feet fall hazard by the use of a personal fall protection system.
