On or about May 30th, 2014, at the above addressed jobsite, in the header area, employees were exposed to amputation hazards while working with a 3/16 National Header machine that had not been provided with a guard. b. On or about May 30th, 2014, at the above addressed jobsite, in the header area, employees were exposed to amputation hazards while working with a 3/16 National Header machine that had not been provided with a guard. c. On or about May 30th, 2014, at the above addressed jobsite, in the header area, employees were exposed to amputation hazards while working with a 3/16 National Header machine that had not been provided with a guard. d. On or about May 30th, 2014, at the above addressed jobsite, in the header area, employees were exposed to amputation hazards while working with a 3/16 National Header machine that had not been provided with a guard. e.
