29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazards created by in-going nip points, rotating parts, and/or flying chips and sparks: a) On or about November 15, 2024, at the tool and production areas of the jobsite located at 7590 W 19th Court, Hialeah, Florida, 33014, employees were exposed to an amputation hazard while two tubular rivet header machines (i.e. National - Model No. 34Y -Serial No. 31702 and 35490 ) were operated without guards. b) On or about November 15, 2024, at the tool and production areas of the jobsite located at 7590 W 19th Court, Hialeah, Florida, 33014, an employee was exposed to an amputation hazard while a lathe machine (i.e. Promach - Model No. PL-18 - Serial No 8418001. ) was operated without guards. c) On or about November 15, 2024, at the tool and production areas of the jobsite located at 7590 W 19th Court, Hialeah, Florida, 33014, an employee was exposed to an amputation hazard while operating a vertical saw (i.e. Rockwell - Serial: 49-963) without guard.
