29 CFR 1926.451(e)(9)(iv): Cross braces on tubular welded frame scaffolds were used as a means of access or egress: (a) On June 6, 2014, at a jobsite located at 200 E 23rd St - Independence, MO., Employees were exposed to serious fall injuries, in that workers were observed traversing and accessing the scaffold while using the frame and cross braces of the scaffold. As a result, employees were exposed to falls up to approx. 15 feet above the ground level. Abatement certification for this violation is not required.

As a result, employees were exposed to falls up to approx. 20 feet above the ground level. Abatement certification for this violation is not required.

29 CFR 1926.454(a)(3): The employer shall have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. The training shall include the proper use of the scaffold, and the proper handling of materials on the scaffold. (a) On June 6, 2014, at a jobsite located at 200 E 23rd St - Independence, MO., Employees were exposed to serious injuries, in that workers displayed inadequacies in training which included the proper use of the scaffold, proper access means and the use of materials and ladders on scaffolds. Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.
