29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) 405 EVANS AVENUE, WYOMISSING PA - Employees performed roofing work at heights ranging from approximately ten (10) to fifteen (15) feet, and the employees were not protected by fall protection, thereby exposing employees to serious injuries, on or about June 4, 2014. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: (a) 405 EVANS AVENUE, WYOMISSING PA - Employees accessed the roof by way of a Werner 24 foot extension ladder, and the rungs of the ladder did not extend a minimum of three feet above the landing surface, thereby exposing employees to serious injury, on or about June 4, 2014. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
