29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment: a) Building C - Operations employees, cleaning out the inside of a tank with a hose, were not qualified to lock out the equipment and maintenance personnel did it for them, on or about July 1, 2014.

29 CFR 1926.850(e): It had not been determined if any type of hazardous chemicals, gases, explosives, flammable materials or similar dangerous substances had been used in any pipes, tanks, or other equipment, on the property. The presence of such substances were apparent and the hazard was not eliminated before demolition was started: a) Outside Old Production Building - No testing or purging had been completed on a 3 inch pipe, that contained residual flammable citrus oil, prior to the removal of the pipe, on or about May 24, 2014.
