29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:    a) Each employee assigned to clean floors, counters and other surfaces within Memorial Sloan Kettering Cancer Center in Basking Ridge, NJ was not informed of the requirements of the OSHA Chemical Hazard Communication Standard; nor was training provided on the details of the employer's Hazard Communication Program, including the location & availability of the programs and explanation of hazard information on container labels.    Condition(s) were noted on or about 7/1/2014.

29 CFR 1910.1030(g)(2)(ii)(A): The employer did not ensure that training was provided to employees with occupational exposure at the time of initial assignment to tasks where occupational exposure might take place:    a) Contract cleaning staff employed by ABM Industries Inc. perform janitorial duties at Memorial Sloan Kettering Cancer Center in Basking Ridge, representing occupational exposure to bloodborne pathogens, with employment tenures ranging from 3 months to 4 years.  Such duties include disinfection of floors and other work surfaces which can be contaminated with blood, as well as collection & disposal of waste from medical treatment area(s) / room(s).  No information or training was provided by the employer to covered employee(s) prior to "Bloodborne Pathogens Safety" hand-out or after 5/19/2014; and this bulletin provided no guidance on procedure to follow for an exposure incident.     Condition(s) were noted on or about 6/03/2014.
