Located at Picatinny Arsenal, New Jersey: The employer did not record the following workplace injury on the OSHA 300 Log for calendar year 2012: a) On or about 11/15/12 an employee had surgery for carpal tunnel syndrome resulting in 10 days away. Violation occurred on or about 1/28/14. Located at Picatinny Arsenal, New Jersey: The employer did not record the following workplace injuries on the OSHA 300 Logs for calendar year 2013: a) On or about 1/2/13 an employee fell, received cracked ribs and had 60 days away. b) On or about 12/13/13 an employee fell on ice, receiving lacerations to the head and had 4 days away. Violation occurred on or about 1/28/14.
