This violation occurred on or about June 2, 2014, near Blue Heron Pond located at 2102 North Trade Days Boulevard in Canton, Texas 75103, where an employee who was working on a recirculation pump that was not isolated from energy sources and rendered inoperative was electrocuted. The employer had not established a program consisting of energy control procedures, employee training and periodic inspections to ensure control of hazardous energy.
