29 CFR 1926.1051(a): A stairway or ladder shall be provided at all personnel points of access where there is a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist is provided: a. On or about June 05, 2014, at the above addressed jobsite, a safe means of access was not provided in that employees had to walk across a plank, four (4) feet ten (10) inches above the ground, to access the building thereby exposing employees to fall hazards.
