29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:    a) Die Clean Room: The employer failed to ensure that a protective apron or other means to protect the outer extremities was provided and utilized by employees performing die clean operations utilizing caustic soda. No impervious gear was utilized by operators while a contact exposure existed.

29 CFR 1910.132(e): The employer did not ensure that employees were not using defective or damaged personal protective equipment in the workplace:    a) Die Clean Room: The employer failed to ensure that hand protection was not deflective and/or damaged for employees performing die clean operations utilizing caustic soda. An employee was utilizing a cotton/flex, wrist length gloves that were torn near the finger tips exposing the worker to chemical burns.

29 CFR 1910.138(a): The employer did not select and require employee(s) to use appropriate hand protection when employees' hands were exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasion; punctures; chemical burns; thermal burns; and harmful temperature extremes     a) Die Clean Room: The employer failed to ensure that adequate hand protection was provided and/or utilized by a die clean operator while performing dipping operations, cleaning, waste removal, adding solvent to the caustic tank and other duties involving the use of  sodium hydroxide. An employee was utilizing a cotton/flex, wrist length glove while performing dipping operations.

29 CFR 1910.303(b)(6): Conductors or equipment were located in damp or wet locations; where exposed to gases, fumes, vapors, liquids, or other agents that had a deteriorating effect on the conductors or equipment; or where exposed to excessive temperatures and were not identified for use in the operating environment:     a) Die Clean Room: The employer failed to ensure that all electrical equipment operated in the area was not affected by deteriorating agents. Caustic soda utilized to clean dies from the extrusion process exposed the electrical exhaust fan to the material causing the motor to short circuit. Employees were exposed to a fire hazard while dipping parts into a tank of caustic soda.
