29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames,  or uprights did not bear on base plates, mud sills or other adequate firm foundation:    a- 18 East Rutherford Avenue, Lavallette NJ: An employee, not supplied with fall protection, was exposed to fall hazards of approximately 10 feet 7 inches to the ground surface below when performing siding installation and other related activities from a pump jack scaffold whose legs were set and sinking into the sand surface below. Condition observed on or about 06/11/14.

29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration:; Such activities were not performed only by experienced and trained employees selected for such work by the competent person.    a- 18 East Rutherford Avenue, Lavallette NJ: An employee, not supplied with fall protection, was exposed to fall hazards of approximately 10 feet 7 inches to the ground surface below when performing siding installation and other related activities from a pump jack scaffold whose walking/workiing surface was missing top and midrails as well as the end stops and the scaffold was not set on a stable surface. The worksite company employees had not received any scaffolding training from the company.  Condition observed on or about 06/11/14.     NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
