29 CFR 1910.184(c)(1): Sling(s) that were damaged or defective were used: . (a) On or about June 13, 2014, in the facility, the safety latch on a hook of the metal sling was missing, and had been replaced with a piece of wire, in lieu of a manufactured safety latch. This metal sling was attached to the Clark fork lift truck being used to move concrete products around the facility. (b) On or about June 13, 2014, in the facility, safety latches on two hooks of the metal slings were missing. These metal slings were attached to the Clark fork lift truck being used to move concrete products around the facility.
