29 CFR 1910.22(b)(1): Aisles and passageways were not kept clear and in good repair, with no obstruction across or in aisles that could create a hazard:  a) Top of Stairs On Winder Level - Employees entering and exiting the upper level "Winders" were exposed to struck by hazards from Winder 28 and Winder 32 rotating into the aisle, on or about June 12th, 2014.  b) Top of Stairs on Winder Level - Employees entering and exiting the upper level "Winders" were exposed to struck by hazards from Winder 8 and Winder 12 rotating into the aisle, on or about June 12th, 2014.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  a)  Upper Level Winders - Employees changing out rolls on the twenty two (22) Kiefel 113mm Line machines were exposed to point of operation hazards from unguarded rotating parts, on or about June 12th, 2014.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
