29 CFR 1910.22(a)(1): (1) All places of employment, passageways, storerooms, service rooms, and walking-working surfaces were not kept in a clean, orderly, and sanitary condition:  a. On February 1, 2017, in the extruder department, the catwalk platform, rails, building girders, beams, pipes, and other machinery were not kept clean from plastic dust accumulation.  Hilex Poly Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.22(a)(1), which was contained in OSHA inspection number 1101914, citation number 2, item number 1 and was affirmed as a final order on January 8, 2016, with respect to a workplace located at 2800 Sprouse Drive, Richmond, Virginia.

29 CFR 1910.37(b)(5): Each doorway or passage along an exit access that could be mistaken for an exit was not marked "Not an Exit" or similar designation, or be identified by a sign indicating its actual use (e.g. closet):  a. On February 1, 2017, on the north side of the building, the four doors that access the rail cars were not labeled "not and exit".
