29 CFR 1910.244(a)(2)(i): In the absence of a firm foundation, the base of the jack was not blocked: Workplace: Neither the base of the hydraulic jack, used by employees during the process of welding a tank was placed on a firm foundation nor was blocked appropriately. On/about 6/9/2014, the base of the jack slipped off during the operation, and the piston of the jack flew out and struck an employee's head. The employee sustained serious injury from the incident.
