29 CFR 1926.651(j) (2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary The employer does not ensure that protection was provided by placing and keeping excavated or other materials or equipment at least 2 feet from the edge of excavations. This violation was observed on or about June 12, 2014, when employees working in an excavation were exposed to struck-by hazards from the excavated material that was not at least 2 feet from the excavation edge. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that protection is provided by placing and keeping excavated or other materials or equipment at least 2 feet from the edge of excavations.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 paragraph (b) or (c): The employer does not ensure that each employee in an excavation was protected from cave-ins by an adequate protective system. This violation was observed on or about June 12, 2014, when employees were exposed to cave-in hazards while doing utility work in an excavation that was not protected from cave-ins. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that each employee in an excavation is protected from cave-ins by an adequate protective system.
