29 CFR 1910.333(c)(3)(iii)(A): Vehicles or mechanical equipment capable of having parts of their structures elevated near energized overhead lines were not operated so that a clearance of 10 feet (305 cm) plus 4 inches (10 cm) for every 10 kV over 50 kV was maintained: (a) Ranger Station site in Prentice, WI; An arc over occurred when a backhoe was operated within 10 feet of an energized overhead transmission line.

29 CFR 1910.335(b): No alerting techniques were used to warn and protect employees from hazards which could cause injury due to electric shock, burns, or failure of electric equipment: (a) Construction site located Prentice, WI; Attendant(s), barricades, signs, symbols, or markings were not provided to warn against hazards presented by an overhead 115 kV electric transmission line while the backhoe was being used to lift the rear of a dump truck in order to change a tire.
