29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could have resulted in possible cave-ins, failure of protective systems or other hazardous conditions: (a) Excavation in the vicinity of W5327 Prentice Ranger Station, Prentice WI; The daily inspection of the trench was not conducted by a competent person as the employees were working in an unprotected trench without adequate cave in protection. The competent person did not display effective knowledge of safe excavation requirements and employees were working in a trench that had water infiltration and soil falling from the edges during the inspection.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c): (a) Excavation in the vicinity of W5327 Prentice Ranger Station, Prentice WI; Employees were unprotected while working in a trench that did not have adequate cave-in protection systems. The trench was approximately six feet two inches deep with an excavation face that was sloped at 56 to 82 degrees.
