29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables shall not be used. On or about June 12, 2014 employees were exposed to two instances of electrical hazards. 1. Electrical extension cord is frayed exposing wires. 2. Electrical power cord to the circular saw has black electrical tape being permanently used to repair torn power cable.

29 CFR 1926.20(b)(1): It shall be the responsibility of the employer to initiate and maintain such programs as may be necessary to comply with this part. On or about June 12, 2014, the employer did not provide any verbal safety training or awareness to employees in such that the employees were not wearing any eye protection while using a Stihl metal cutting saw. Employees were also unaware of the safety hazards in the use of electrical equipment with frayed and taped cords.
