OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees employees in that employees were exposed to the hazard of falling from and being struck-by moving equipment:  (a)Rocky Mountain Inspection Services, Inc. @ 369 East 8th Street, Greeley, CO 80631:  On and before June 4th, 2014, the employer did not ensure employees were not riding on equipment not equipped for riders while in operation. On May 20th, 2014, an employee attempted to jump onto the step of a 950G CAT front-end loader while in operation. The employee missed the step and fell off the loader and had his right leg partially ran over by the rear wheel. This condition exposed the employee to crushing hazards.  Abatement Note: Among other methods, one feasible and acceptable abatement method to correct this hazard can be found in SAE J153, Surface Vehicle Recommended Practice, Section 4.3 Operator-General Precautions, instructs operators: "Do not permit riders on the machine if there are no manufacturer's designated place for the rider." In addition, Section 7.10 Operator- General Precautions, instructs operators: "Watch for bystanders and never allow anyone to be under or reach through the machine and it's equipment while operating."
