OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by hazards: (a) On or before March 8, 2025, while in the process of conducting the hydrostatic testing of a P110 tube, the employer failed to ensure that the operator and other employees in the immediate vicinity were protected from inadvertent struck-by hazards. The P110 tube was pressurized to approximately 7500 PSI when the high-pressure hose unexpectedly disconnected striking an employee, who was assisting with the pressurization testing, resulting in multiple injuries.

29 CFR 1904.39(a)(2):The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours: (a) On or before March 8, 2025, the employer failed to report the hospitalization of an employee after a serious injury which occurred at the hydrostatic testing site. The employee sustained injuries on March 8, 2025 at approximately 10:00 AM and OSHA was not notified of the hospitalization until March 10, 2025 at 12:00 PM.
