29 CFR 1926.403(b)(2):Listed, labeled, or certified equipment was not installed and/or used in accordance with instructions included in the listing, labeling, and/or certification:  (a) Westborough Village Development, Westborough, MA: On or about March 14, 2014, employees were using a GFCI outlet contrary to the instructions printed on the device that state it must be plugged into the wall socket; this GFCI was observed in use, plugged into an extension cord to power circular saws and an air compressor.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws:  (a) Westborough Village Development, Westborough, MA: On or about March 14, 2014, the main extension cord used to provide power to the site from the generator exhibited severe signs of strain as the outer jacket of the cord was pulled completely free of the terminal end.

29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:  (a) Westborough Village Development, Westborough, MA: On or about March 14, 2014, several employees were working/walking on the 2nd story floor exposed to falls through the stair well openings of more than 9 ft., and on the main floor falls into the basement stair well openings of more than 8.5 ft. due to the lack of covers and/or railings.  Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).

New Stars Construction Group , LLC was previously cited for a violation of this occupational safety and health standard  29 CFR 1910.178 (l) which was contained in OSHA inspection number 953417, citation number 1, item number 1 and was affirmed as a final order on March 8, 2014, with respect to a workplace located at 32 Hopkins Circle, Methuen, MA.  Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).

New Stars Construction Group , LLC was previously cited for a repeat violation of this occupational safety and health standard  29 CFR 1926.501 (b) (13) which was contained in OSHA inspection number 953417, citation number 2, item number 1 and was affirmed as a final order on March 8, 2014, with respect to a workplace located at 32  Hopkins Circle, Methuen, MA.  Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).

New Stars Construction Group , LLC was previously cited for a violation of this occupational safety and health standard  29 CFR 1926. 1053 (b) (1)  which was contained in OSHA inspection number 689318, citation number 1, item number 3 and was affirmed as a final order on November 1, 2012, with respect to a workplace located at 143 Lilac Ln., Prospect, CT.  Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).
