29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration:   a) At the jobsite, on or about 5/7/14:  Employees were using a tubular welded frame scaffold where the platform was over 10 feet from ground and where the upper work platform was not fully planked.  Workers on this scaffold were not provided fall protection and the scaffold was built with mixed components that are not designed for this purpose.   b)  At the jobsite, on or about 5/7/14:  Employees were using a ladder jack scaffold, approximately 11 feet from ground, without fall protection, and without adequate access to the work platform.         Abatement certification must be submitted for this item.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.  a)  At the jobsite, on or about 5/7/14:  Employees were using a ladder jack scaffold, approximately 11 feet from ground, without fall protection, and where the work platform was mounted outside the ladders, causing employees to climb over the pick.         Abatement certification must be submitted for this item.

Workers on this scaffold were not provided fall protection and the scaffold was built with mixed components that are not designed for this purpose.  The scaffold was built with the work platform in close proximity to a power connection to the building and employees were not trained to recognize or minimize those hazards.     b)  At the jobsite, on or about 5/7/14:  Employees were using a ladder jack scaffold, approximately 11 feet from ground, without fall protection, without adequate access to the work platform, and employees were not trained to recognize or minimize those hazards.         Abatement certification must be submitted for this item.
