29 CFR 1926.501(b)(11): "Steep roofs." Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. On or about October 10, 2017, during the installation of wood decking/sheeting material for the construction of a assisted living facility, located at 26695 Center Ridge Road, Westlake, Ohio, the employer did not ensure three employees working from a 5/12 pitched roof were protected from a fall of approximately 10.5 feet, by utilizing a guardrail system, safety net system, and/or personal fall arrest system.
