29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: On or about June 17, 2014, at the above addressed jobsite, employees working below residential roofing operations were not wearing hard hats to protect against falling material and debris. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about June 17, 2014, at the above addressed jobsite, employees engaged in residential roofing operations, twenty-one (21) feet three (3) inches above the ground, were not protected from falling, thereby exposing employees to fall hazards. The Keith Tilden Roofing Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 929713, citation number 2, item number 1 and was affirmed as a final order on March 4, 2014, with respect to a workplace located at 223 N. Monroe, Hinsdale, IL 60521. The Keith Tilden Roofing Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 907113, citation number 1, item number 2 and was affirmed as a final order on June 26, 2013, with respect to a workplace located at 4558 Grand Ave., Western Springs, IL 60558. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
