29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. On December 10, 2014 employees were not protected from falling through skylights by protective covers or a personal fall arrest system, exposing employees to a fall of over six feet. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

Keith Tilden Roofing Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13) which was contained in OSHA inspection number 981061.015, citation number 2, item number 1, and was affirmed as a final order on September 17, 2014, with respect to a workplace located at 475 Thurlow St. Hinsdale, IL 60521.
