OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and crushed-by hazards created by metal storage rack systems with damaged bottom columns and cross braces. a) Warehouse, 4th Street/Pacific Avenue, West Pittston, PA: Employees stored/retrieved records on five (5) metal storage rack systems in the 200's, 300's and 500's sections of the warehouse that had bottom columns and cross braces that were bent and damaged. Employees were exposed to struck-by and crushed-by hazards in the event of a metal storage rack system collapse, as observed on or about June 27, 2014. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.

29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards. a) Warehouse, 4th Street/Pacific Avenue, West Pittston, PA - Employees operated a Crown SP3500 Stockpicker to aid in the delivery and retrieval of storage boxes on an elevated platform and wooden pallets above ground level that had unguarded open-sides. Employees were exposed to fall hazards greater than four-foot (> 4') onto the concrete warehouse floor, on or about June 18, 2014. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.

Employees were exposed to struck-against hazards due to a lack of lighting at or above 2 lumens per square foot, as observed on or about June 18, 2014. In accordance with 29 CFR 1903.19(c), abatement certification is not required for this violation. Abatement was observed during the inspection.
