29 CFR 1904.7(b)(3): The employer did not correctly classify the following work-related injuries or illnesses on the OSHA Form 300 or equivalent:  The 2014 OSHA 300 log provided to OSHA on June 17, 2014 contained the following errors:      a) Case number 14_F_011, with an incident date of 02/05/2014, had a check mark in column J (Other recordable cases). The employee was still being medically treated with over 140 lost work days and 13 restricted work days.  b) Case number 14_F_013, with an incident date of 02/12/2014, had a check mark in column J (Other recordable cases). The employee had been medically cleared on 04/02/2014 with 40 lost work days and 8 restricted work days.  c) Case number 14_F_022, with an incident date of 03/03/2014, had a check mark in column J (Other recordable cases). The employee had been medically cleared on 04/19/2014 with 14 restricted work days.  d) Case number 14_F_027, with an incident date of 03/12/2014, had a check mark in column J (Other recordable cases). The employee was medically cleared on 05/22/2014 with 5 lost work days and 51 restricted work days.  e) Case number 14_F_054, with an incident date of 05/05/2014, had a check mark in column J (Other recordable cases). The employee was still being medically treated with over 20 lost work days.  f) Case number 14_F_057, with an incident date of 05/19/2014, had a check mark in column J (Other recordable cases). The employee had been medically cleared on 05/28/2014 with 2 lost work days and 6 restricted work days.
