29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. (a)Advanced Roofing Technologies, Ltd at 255 E Sixth St., Loveland CO, 80537: On and before June 17th, 2014, the employer did not ensure employees were protected from fall hazards while conducting roofing operations. Employees were observed installing flashing material outside of the warning line system without a safety monitor present observing employees. The condition exposed employees to a fall hazard of approximately 20 feet to the ground below while installing flashing material along the perimeter of the flat roof.

29 CFR 1926.502(h)(1)(v): The safety monitor had other responsibilities which could take the monitor's attention from the monitoring function: (a)Advanced Roofing Technologies, Ltd at 255 E Sixth St., Loveland CO, 80537: On and before June 17th, 2014, the employer did not ensure that the employee performing the duties of a safety monitor did not perform work activities that would interfere with the function of ensuring roofing employees were not exposed to fall hazards. Employees were observed conducting roofing operations outside the warning line system without the presence of a safety monitor. The condition exposed employees to a fall hazard of approximately 20 feet to the ground below while installing flashing material along the perimeter of the flat roof.
