29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. On or about June 18, 2014 and times prior thereto, employees of Woodworks Contractors, Inc. were observed framing the two story residence being constructed at or near 5345 Chateau Drive in Conway, Arkansas. The employees used nail guns during framing of the dormer without the use of eye protection, exposing employees to eye injuries.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about June 18, 2014 and times prior thereto, employees of Woodworks Construction, Inc. were observed framing a two story residence being constructed at or near 5345 Chateau Drive in Conway, Arkansas. Employees worked on the 2x6 framing members and the partially decked rooftop with a pitch of approximately 10:12 without the use of fall protection, exposing employees to falls greater than 10 feet to the ground level below.

29 CFR 1926.1053(b)(13): The top or top step of a stepladder was used as a step: On or about June 18, 2014 and times prior thereto, an employee of Woodworks Contractors, Inc. was observed assisting in framing of the two story residence being constructed at or near 5345 Chateau Drive in Conway, Arkansas. The employee was standing on the top step of a Werner A-frame stepladder with model number MT-22, exposing the employee to fall hazards.
