29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    On or about June 18, 2014 and times prior thereto, employees of Jose Ibarra performed decking of a steep sloped rooftop for a residence being constructed at or near 3920 Riley Renee Cove in Conway, Arkansas.  An employee was observed working from ground level was exposed to possible danger of head injury from impact of objects used on the rooftop to include but not limited to nails, nail gun, and plywood.  The employee was observed working without the use of a protective helmet.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.    On or about June 18, 2014 and times prior thereto, employees of Jose Ibarra were observed decking a steep sloped rooftop of a residence being constructed at or near 3920 Riley Renee Cove in Conway, Arkansas.  Employees did not use eye protection when decking with the use of a nail gun and wood cutting operations, exposing employees to eye hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     On or about June 18, 2014 and time prior thereto, employees of Jose Ibarra performed decking on a steep sloped rooftop of a residence being constructed at or near 3920 Riley Renee Cove in Conway, Arkansas.  An employee was observed walking and working on the rooftop without the use of fall protection, exposing the employee to fall hazards greater than 6 feet.
