29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. a) On or about 6/16/14 outside of Texas deBrazil Steakhouse building under renovation at 1 Walden Galleria, Cheektowaga, NY: An employee removing bags of debris from an opening in the building wall was exposed to a potential fall of approx. 13 ft. while in the elevated workplatform on a Skyjack, Model SJIH 4632 Scissors Lift. The "mid-chain" on the end of the lift was not fastened. NO ABATEMENT CERTIFICATION REQUIRED
