29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)     a). South side of structure- Employees performing stucco preparation work were exposed to fall hazards of approximately twenty (20) to thirty (30) feet without the walking working platform of the scaffold being fully planked or decked between the front uprights and guardrail supports.  Observed on or about 17 JUNE 2014.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames,  or uprights did not bear on base plates, mud sills or other adequate firm foundation:  a). South side of structure- The poles of the scaffold were placed on the soil directly without the use of a baseplate nor adequate firm foundation, and, in one instance, placed on scaffold planks above a basement window well where the poles did not reach the ground. Observed on or about 17 JUNE 2014.  b). South side of structure- Employees installing wire mesh for stucco application were exposed to a scaffold collapse hazard as the tubular welded frame scaffold did not bear on base plates or a firm foundation. Observed on or about 17 JUNE 2014.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.  a). South side of structure- Employees were not provided with a safe means of access and/or egress to the tubular welded frame scaffold platform from a lower level, exposing employees to a fall hazard of approximately twenty-nine (29) feet.  Employees were using the side frame of the scaffold to access and/or egress the working levels.  Observed on or about 17 JUNE 2014.
