29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)   a) Jobsite - Employees performing stucco preparation work were exposed to fall hazards of approximately twenty (20) to thirty (30) feet without the walking working platform of the scaffold being fully planked or decked between the front uprights and guardrail supports.  Observed on or about 15 SEP 2015.    Cristian Sanchez dba MSJ Contractors, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 981415, citation number 1, item number 1 and was affirmed as a final order on 08/13/2014 with respect to a workplace located at 1826 Norris Avenue, Philadelphia, PA.

Observed on or about 15 SEP 2015. 29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.  a) Jobsite - Employees were not provided with a safe means of access and/or egress to the tubular welded frame scaffold platform from a lower level, exposing employees to a fall hazard of approximately thirty (30) feet.  Employees were using the side frame of the scaffold to access and/or egress the working levels.  Observed on or about 15 SEP 2015.
