All personal protective equipment shall meet the requirements and be maintained according to Subparts D and E of this part. (a) On June 19, 2014, at the job site located at 1722 Glenstone Ave, Springfield, MO., Employees engaged in exterior building repair activities were exposed to eye hazard injuries, in that they were not using any eye protection while using power tools to attach wooden shingles. Abatement certification for this violation is not required.

(a) On June 19, 2014, at the job site located at 1722 Glenstone Ave, Springfield, MO., Employees engaged in exterior building repair activities were exposed to fall hazard injuries from both a scaffold and unprotected sides and edges of a building roof. Inadequacies in the employees knowledge and use of fall protection systems for scaffolds and working levels above 6 feet, indicate that the employees did not retain the required understanding and skill to work safely and utilize appropriate fall protection measures. Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose. (Please ensure that employees have completed appropriate fall protection training or refresher training to abate this citation)
