29 CFR 1926.20(b)(1): The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:  A) Absolute Roofing on or about May 31, 2019 at the address of 1659 Bellefontaine St. Wapakoneta, Ohio: The employer did not ensure that a safety and health program was initiated to identify job site hazards which assist employees maintaining a safe working environment. Such programs  would support the employer and employees in identifying and avoiding potential hazards such as falls during jobsite operations and decreasing the probability of employee injury and/or illness. Employees were exposed to falls off approximately 7 1/2 feet to the ground below during re-roofing operations.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   A) Absolute Roofing on or about May 31, 2019 at the address of 1659 Bellefontaine Street, Wapakoneta, Ohio: The employer did not ensure employee's had been trained in the appropriate use of personal fall arrest systems or alternative measures while performing work tasks from elevated surfaces. Employees were exposed to falls of approximately 7 1/2 feet to the ground below during re-roofing operations.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems:  A) Absolute Roofing on or about May 31, 2019, at the address of 1659 Bellefontaine Street Wapakoneta, Ohio: The employer did not ensure employees were protected from falls while tearing off roofing material of a commercial type building. Employees were exposed to an approximate 7 1/2 foot fall to the ground below.
