29 CFR 1910.305(b)(2)(i): Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose:    a) Slitting Area: The employer failed to ensure that a junction box at the slitter uncoiler  (Parker Model D3W1CNYG) was provided with a cover to prevent the interior wires from exposing employees working in the area to shock hazards.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  a) Mill and Slitting Areas: Employees were required to work with chemicals such as, but not limited to Carbon and Low Alloy Steel, Hot Rolled Alloy Steel Sheet, TAFA Tin Alloy Wire, Tube-Alloy Cobalt Based Cored Wires, and Welding gases while forming steel tubes.  The employer did not provide training specific to the hazards of chemical used at the establishment to all employees working with hazardous chemicals.
