29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.    a) Mill Number 1, Welding Area: On or about April 25, 2019, and at times prior, the employer exposed employees to struck-by and crushing hazards, in that the Mill Number 1 was not properly guarded.    b) Mill Number 1, Turks Area: On or about April 25, 2019, and at times prior, the employer exposed employees to struck-by and crushing hazards, in that the Mill Number 5 was not properly guarded.    c) Mill Number 5, Steel Coil Feeding Area: On or about April 25, 2019, and at times prior, the employer exposed employees to struck-by and crushing hazards, in that the Mill Number 5 was not properly guarded.

29 CFR 1910.219(f)(3): Sprockets and chains. All sprocket wheels and chains shall be enclosed unless they are more than seven (7) feet above the floor or platform. Where the drive extends over other machine or working areas, protection against falling shall be provided. This subparagraph does not apply to manually operated sprockets.    a) Mill Number 1, Conveyor Area: On or about April 25, 2019, and at times prior, the employer exposed employees to caught-in and crushing hazards, in that a chain and sprocket on the Mill Number 1 was not properly guarded.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and cables may not be used as a substitute for the fixed wiring of a structure.  a) Production Area, Mill Number 5: On or about April 25, 2019, and at times prior, the employer exposed employees to electrical shock and fire hazards while utilizing an unsecured 115 volt electrical outlet, which was not connected to the fixed wiring of the building structure.
