29 CFR 1904.29(b)(3): Each recordable injury or illness was not entered on the OSHA 300 Log and/or an incident report (OSHA Form 301or equivalent) within seven (7) calendar days of receiving information that a recordable injury or illness has occurred: Smithfield Farmland Corp. @ 200 S 2nd Street, Lincoln, NE 68508: The employer is failing to record on either the log of all Work-Related Injuries and Illnesses (OSHA Form 300), and/or the Injury and Illness Incident Report (OSHA Form 301), or their equivalents, work-related injuries or illnesses for the calendar year 2014, where at least four employees received medical treatment beyond first aid in the form of therapeutic exercise and physical therapy. On or about 05/27/14, an employee sustained a right leg injury. As a result, the employee received medical treatment beyond first aid in the form of therapeutic exercise and physical therapy and the injury was not recorded. On or about 06/17/2014, an employee slipped, fell and sustained multiple injuries to the right arm, left elbow and left back and hip. As a result, the employee received medical treatment beyond first aid in the form of therapeutic exercise and physical therapy and the injury was not recorded. Abatement Certification and Documentation is required. The abatement certification sheet is enclosed with the citations with the citations.
